Enrolment Forms

 

Thank you for your interest in Mount Lawley Senior High School.  We welcome all eligible applicants from within our local community and look forward to sharing your educational journey.

 

Enrolment Process

 

Stage 1 – Application for Enrolment
 

Mount Lawley Senior High School welcomes Year 7 and 8 enrolment applications for all prospective students residing in the local intake area.

 

To be successful in your application the following criteria must be met:

  • Your usual place of residence is within the school's local intake area,
  • You are an Australian citizen, permanent resident, or hold an approved temporary residence visa,
  • All requested original documentation must be sighted and photocopies provided for us to keep on file,
  • Your completed application must provide true, complete, and accurate information, and
  • A signed copy of your application and all supporting documentation must be submitted by 27 June 2025.

 

A parent or guardian lodges an Application for Enrolment form with the school. The Principal then considers the information provided and determines if your application is approved.

 

You will be notified in writing of the outcome of your application by mid-August 2025.

Stage 2 - Enrolment
  If your application for enrolment is successful, you will be required to complete a Stage 2 enrolment form.   Further documentation may be required to support an appropriate education program for your child.

Middle School

Middle School Application for Enrolment

This form should be used for student enrolments in Year 7 and 8.

Upper School

Upper School Application for Enrolment

This form should be used for student enrolments in Year 9 and 10.

Senior School

Senior School Application for Enrolment

This form should be used for student enrolments in Year 11 and 12.
